Our History
Founded out of a passion for quality patient care and a desire for a better work-life balance. After years of experience in private practice, we recognized the need to break free from the constraints of traditional practice. Nicole wanted to continue providing quality care to her patients on her own terms with a focus on the wellness side of primary care. This led to the establishment of FMW. We work independently and provide the high standard of care we believe our patients deserve. We are committed to quality, patient-focused healthcare that remains at the core of everything we do.
Meet Nicole Fields
Nicole Fields is a Nurse Practitioner (NP) certified in family medicine. Nicole is focused on providing a holistic approach to healthcare. She creates a partnership between provider and patient, focusing on balancing lifestyle changes and standard medical practice. She has clinical interests in wellness and disease prevention, medical weight loss, lifestyle medicine, women's health/hormones, men’s health/hormones, and sexual health.
Nicole received a Bachelor of Science in Nursing from the University of Southern Maine. She worked as a Registered Nurse in the hospital setting for 6 years before continuing her education at the graduate level. She obtained a Master of Science in Nursing from the University of Phoenix and has been practicing family medicine since 2012. She is board-certified by the American Academy of Nurse Practitioners. A committed advocate for continuous learning and patient-centered care, Nicole is an active member of professional associations, including the Obesity Medicine Association, the American College of Lifestyle Medicine, and The Menopause Society.
Beyond her professional endeavors, Nicole enjoys spending quality time with her family. Whether she's exploring new destinations, engaging in outdoor activities like running and hiking, indulging in a bit of retail therapy, or immersing herself in a good book, Nicole embraces a well-rounded lifestyle. Married with two daughters and two lovable doodles, she extends her passion for health and wellness to helping other families optimize their well-being. We do not accept or participate in insurance plans, but strive to make the payment process straightforward for our patients. However, you may use your insurance for labs or radiology testing that is ordered (if applicable). We accept cash payments through Zelle or Venmo and accept all major credit cards. Payment is due at the time of service.

New patient consults are $300 for all services. Established patient office visits range from $100 to $250 depending on the type of visit. Medical Weight loss pricing varies based on the weight loss plan selected.

This visit is 45-60 minutes to establish care. Complete medical history is obtained and pertinent medical records will be reviewed or requested. Complete blood panel and radiology testing to be ordered if applicable. Blood panel typically ordered: all wellness labs, screening for diabetes, screening for thyroid disease, relevant vitamin levels, hormone levels (if needed), and any other labs deemed necessary. After lab review, care plan recommendation will be made. Lab review is included with the initial consult fee.
